PAPERWORK INSTRUCTIONS

• For each document below, fill out ALL required fields and select the "Submit" button.
  *The "Submit" button only appears once all required fields are completed.
• You will receive a confirmation email for every form submitted.
• Time Cards will be sent after event completion.

Payroll Start Form & Guidelines

PLEASE SUBMIT A NEW FORM W-4 BELOW IF:

• You have not yet submitted Form W-4 to One Foot Productions in the year of 2016.
• Your personal or financial information has changed since your most recent 2016 submission.

FORM W-4: EMPLOYEE'S WITHHOLDING ALLOWANCE CERTIFICATE

Complete Form W-4 so that your employer can withhold the correct federal income tax from your pay. Consider completing a new Form W-4 each year and when your personal or financial situation changes.

CONFIRMATION

Thank you for your paperwork submissions. You will now receive separate confirmation emails for each form submitted:

• "OFP Start Form [2016]"
• If applicable: "Form W-4 [2016]"
